To sign up for the app, go to the app’s website. On the home screen, click “Log In” or “New User? Click Here to sign in” if you’ve never used the app. On the sign-up screen, enter your email and the password you would like to use. Click “Sign Up”
On the sign-up page enter your name, phone number, and select your department. Click “Finish” to go to the home page.
If you’ve been sent an invitation to your email to join the app, it means that a temporary account has been made on your behalf. Click on the link in the email invitation to set a password. Once set, you can use your email and newly established password to log in on the home screen.
Click on the “Start Daily Screening” button to initiate the daily COVID-19 questionnaire. Choose the location you will be working today. If you are going to multiple areas, choose “Multiple Locations”. Answer the yes/no questions as they pertain to you on this day.
At the end of the questionnaire, click “Finish”. If you do not want the result emailed to you, uncheck the “Email Result” checkbox.
If one of your answers from the questionnaire indicate that you should stay home, a popup will show asking if you wish to continue or start over. If you feel you may have clicked an answer wrong, click “Start Over” to complete the questionnaire again. Otherwise click “Continue”.
After you have completed the screening questionnaire, you will either get a red “Do Not Go To Work” screen or a green pass screen.
Use the “Start Daily Screening” button to fill out the daily COVID-19 screening questionnaire.
If your production is doing COVID-19 testing, your next scheduled COVID-19 test will be displayed on your home screen.
Any changes to your test (location, timing) will be displayed in real-time on your home screen. Be sure and double-check what’s displayed to get the most accurate information.
If you’ve completed COVID-19 testing, details of the most recent negative test will be displayed for your reference.
The production can use the app to have you complete a quiz to show that you understand the COVID-19 safety protocols. You can find the quiz in the app’s menu. Open the menu by clicking the “magic button” on the bottom of the screen.
If the quiz is mandatory, you will have to complete the quiz prior to being able to use the app to do your daily COVID-19 Screening.
Once the quiz has been completed, the option to take the quiz will no longer be in the menu.
In order to limit the amount of queuing-up in inclement weather, or to prevent congregating around the catering trucks, Production may use the app to have you pre-order your lunch. Click on the “Choose Your Lunch” button to place your order.
To view your profile information (Name, phone number, department) choose “Profile” from the menu. You will also see a history of your last few screening form submissions. Click on the edit icon to make any changes to your profile.
If you get a red screen, it means you should not come to work. Instructions on what to do next will be on the screen. The COVID safety team will receive a notification that you have received a red screen and may be in contact about next steps. If you received a red screen in error, click the “Start Daily Screening” button on your home screen to do the questionnaire again.
On the green screen will be your name, time and date the screening was completed, and a QR code. You will also receive an email with the same information, including in image of the QR code. Show this screen to the COVID safety coordinator to gain admission to the work site.
